Clinical trials of type 2 diabetes patients management solution

Thessaloniki, 31 January, 2019 – The €756,000.00 offer led by GNOMON INFORMATICS SA was qualified with the highest score in the final phase of the Proempower action aiming at the clinical certification of its solution in four countries and 200 patients. Gnomon Informatics SA has developed the solution in collaboration with distinguished collaborators in the field of diabetes management in Europe. The DM4all consortium, led by GNOMON INFORMATICS SA, consists of the Information Technologies Institute (ITI) of the Center for Research and Technology Hellas (Greece), ERC SA from Turkey, Promptly Lda from Portugal, Medisanté AG from Switzerland and GPI SpA from Italy (https://proempower-pcp.eu/pcp-contractors/dm4all.html).

Diabetes is a chronic disease that affects 9.1% of the adult population of Europe according to the International Diabetes Federation (IDF) diabetes atlas with an increasing trend in 10.7% of the population by 2040. Patients with diabetes require not only effective treatment, but also continuity in their care, as well as adequate information and support so that they can self-manage their disease to the maximum extent possible.

With the implementation of the DM4all solution on a large scale, Gnomon Informatics SA aspires to upgrade the health value chain through the use of innovative tools, launching a new data value chain to improve collaboration among stakeholders in managing diabetes (patients, healthcare professionals, carers, relatives, healthcare services buyers and the state). The solution is based on powerful data exchange and interoperability bases that enable people, software and medical devices to communicate continuously and securely. The solution is aiming at integrating the best possible tools to enable patient self-management as well as the decision-making process.

DM4all system will be clinically validated over a period of 16 months at 4 diabetes centers in Europe and more specifically in Turkey at Ümraniye Eğitim ve Araştırma Hastanesi Hospital in cooperation with the Turkish Ministry of Health, in Italy at the Azienda Ospedaliera Universitaria Federico II in Naples, in Spain in the Servicio Murciano de Salud and in Portugal at the Santa Casa da Misericórdia da Amadora Center in Lisbon with the support of the state health agency Servicos Partilhados do Ministerio de Saude (SPMS) of the Ministry of Health of Portugal. The DM4all solution consists of:

An interoperability subsystem (API) based on HL7 FHIR standards and IHE integration profiles for the continuous and secure data exchange with existing third-party applications and medical devices
An Internet application and a mobile application (mHealth) for end users
A shared care plan management application
Clinical display tools
Clinical decision support system
A virtual agent application to help patients manage their disease
A Dataset Unification Middleware (DUM) and clinical concepts application
A Diabetes Patient Outcomes Management System (PROMs)
A patients community management portal
A medical devices management and diabetes parameters telemonitoring platform in cloud (Internet of Medical Things)
A Drug to Drug Interaction management service (DDI)
More specifically, DM4all solution is based on Gnomon Informatics SA ehealthPassTM platform that enables continuous data integration and exchange with third-party applications and medical record management applications based on international standards (HL7 FHIR, IHE integration profiles, etc.), complex clinical processes and common therapy plans creation, collaboration and interaction between patients and health professionals, providing among others their consent to their medical data management, as well as information and measurements uninterrupted flow from various medical devices in cloud (Internet of medical things).

Gnomon Informatics SA has implemented IT infrastructures in Europe that enable cross-border provision of medical care between European states as well as chronic diseases management, utilizing modern internet and mhealth technologies, healthcare data privacy assurance and international terminologies and standards. Similar projects have already been implemented or are being implemented by Gnomon Informatics SA on behalf of various health organizations in Greece, Cyprus, Ireland, Malta, Luxembourg, Belgium and Switzerland.
