xShare – Expanding the European EHRxF for Seamless Health Data Exchange

xShare envisions that everyone can share their health data in European Electronic Health Record Exchange Format (EEHRxF) with a click-of-a-button. The xShare Yellow button to be featured across health portals and patient apps, will allow people to exercise their data portability rights under GDPR. Through xSHARE, the EEHRxF will enable everyone to safely share their health data with a click of the xSHARE button and citizen engagement will be the driver for research as new innovative ways to explore health data will truly realize the vision of the European Health Data Space (EHDS).

By harmonizing structured and coded Electronic Health Records (EHRs) across the EU, xShare seeks to enhance data quality, interoperability, and accessibility, promoting the free movement of digital health services. The initiative also aims to standardize health data formats, ensuring their utility in various sectors, including continuity of care, public health, and clinical research. Furthermore, xShare integrates regulatory and industry stakeholders, establishing the European EHRxF Standards and Policy Hub, which will sustainably support and drive digital health innovations across Europe.

The xSHARE project is a major step toward improving health data interoperability in Europe by expanding the EHRxF. Here, we will focus on three key objectives:

The Policy Hub serves as a central governance body for the harmonization, maintenance, and adoption of the EHRxF. It brings together standards organizations, industry leaders, and regulators to ensure interoperability, support business-driven implementations, and monitor adoption across Europe.

The xShare Yellow Button provides a one-click solution for individuals to securely receive, store, and share their health data in compliance with GDPR. The Data Portability Toolkit ensures structured and coded interoperability, integrating seamlessly with health portals, mobile apps, and EHR systems, empowering citizens to control their medical data.

The xShare framework is being piloted in eight European countries, integrating the xShare Yellow Button within national and regional healthcare ecosystems. These sites serve as testing grounds to validate interoperability, ensure compliance with EHRxF standards, and refine real-world applications for cross-border health data exchange.

GNOMON leads Work Package 3 (WP3) – “Patients’ Right to Data Portability: the Case of Continuity of Care”, which focuses on developing the xShare Yellow Button Data Portability Toolkit and ensuring its seamless implementation across the eight adoption sites. As part of this role, GNOMON will implement the xShare Yellow Button in Greece and Ireland, ensuring its integration with national EHR systems and enabling GDPR-compliant health data exchange. In addition to its technical contributions, GNOMON collaborates with health authorities, industry stakeholders, and regulators to drive adoption and long-term sustainability. As a key partner in the European EHRxF Standards and Policy Hub, GNOMON also plays a role in shaping the technical and policy framework for digital health interoperability across Europe!

Project summary

The myHealth@myHands project aims to revolutionise digital healthcare across the European Union by 2030. Our primary objective is to ensure that all EU citizens have secure online access to their medical records, thereby simplifying health management. Thus, the project empowers individuals by granting them control over their health data and allowing them to determine who can access this information, alongside receiving personalised health support.

By integrating existing healthcare infrastructures with MyHealth@EU services, we aim to enhance data interoperability and facilitate seamless healthcare experiences across the EU. Our focus is on developing user-friendly, secure, and flexible eHealth solutions, meticulously designed with a user-centred approach to ensure effective implementation and scaling.

We are also establishing a knowledge hub to disseminate best practices and support collaborative efforts within the healthcare community. The myHealth@myHands project addresses the challenge of fragmented data access, striving to ensure efficient and cohesive healthcare delivery across member states. Through these efforts, we aim to significantly enhance the overall health ecosystem, benefiting all EU citizens.

IRHIS aims to accelerate the upscale of remote health innovations and innovators offering AI-powered, cutting-edge sensor solutions, wearable devices, and remote monitoring solutions across three critical EU remote care areas: musculoskeletal rehabilitation, neurological disorder treatment, and critical patient conditions. By leveraging Big Data, Artificial Intelligence (AI), and Machine Learning, IRHIS will personalise clinical paths and improve patient outcomes while ensuring regulatory compliance and cross-device interoperability.
21 partners from 13 European regions intend to demonstrate a portfolio of 3 interregional investments of 8 technologies developed by EU MedTech SMEs, to jointly boost and deploy innovative remote care solutions, embedding Scale-Ups from transition and less developed regions into global value chains. To that aim, the 3 DemoScale labs will facilitate the demonstration of 8 key TRL6+ technologies and accelerate their faster market uptake. The project IRHIS will federate less developed, transition and developed regions around joint innovation investments and will support the advancement of regional smart specialisation around the shared S3 priority Medical Technology towards commercialisation and scale-up.
IRHIS aims to demonstrate and scale 8 key technologies developed by EU MedTech SMEs, embedding Scale-Ups from less developed regions into global value chains, thereby boosting innovative remote care solutions.
1. Scale Remote Health Innovations: Accelerate the upscale of AI-powered sensor solutions, wearable devices, and remote monitoring technologies.
2. Enhance Musculoskeletal Care: Integrate 3 medical sensor technologies into a remote care platform for personalised rehabilitation.
3. Improve Neurological Treatments: Develop automated rehab technologies to reduce misdiagnoses and enhance prevention for neurological disorders.
4. Advance Critical Care Monitoring: Integrate advanced sensors and AI predictive analytics for ICU remote monitoring and early sepsis detection.
5. Foster Ecosystem Growth: Build a thriving interregional ecosystem for Advanced Remote Care technologies, enhancing market adoption and financial access.
IRHIS will deploy investments related to the use of digital tools to stimulate prevention, enable feedback, and enhance interaction with healthcare providers. This initiative aims to materialize the Internet of Medical Things (IoMT) by creating a network of interconnected medical devices, applications, and health information systems. A key component of this investment strategy involves the establishment of the pan-European DemoScale Lab environment, designed to position IRHIS demonstrators from LDR and initiate the Remote Care Investment Club.
Gnomon is the leader of the Musculoskeletal Care & rehabilitation Demonstration Lab, where three different innovative wearable sensors will integrate to eHealthPass for biomarkers collection and AI-driven insights!

DYNAMO will result in a lean and powerful solution enabling quick, data-driven and platform-independent planning of care pathways for situations where health system functions are threatened. This solution will allow EU health care systems to build resilience and respond to public health threats better than they can today.

DYNAMO brings together four procurers from Italy, Portugal, Spain, Poland and the United Kingdom that not only see the need for such a solution but are willing to act on it now. They will set up a collaborative demandside initiative to create the economies of scale and early adoption cases required to build momentum for DYNAMO. As a strategic planning tool, DYNAMO will guide stakeholders in concept development, data modelling, process design and implementation planning. DYNAMO will be able to reliably inform medium-term investment planning to adapt existing processes and infrastructures (incl. IT systems).

More specifically, DYNAMO will be a socio-technical system to

Efficiently adapt service delivery processes to shocks and structural changes

Share and integrate cross-sectoral data to accurately forecast outcomes and impacts of alternative pathway configurations

Facilitate task planning and skills matching in times of crises The Buyers Group includes health and care organisations and authorities representing diverse health care systems and settings, thus reflecting the requirements of different national health care ecosystems that DYNAMO will have to meet.

Gnomon led the Consortium of the solution “H-CARES” consisted of the following partners: Promptly Health (Portugal & UK), Datawizard (Italy), TREE TECHNOLOGY SA (Spain), Sano (Poland), ICCS (Greece), and Healthink (subcontractor of Gnomon) (Greece)

i2X – Intelligent Implementations of the European EHR Exchange Format

The European Electronic Health Record Exchange Format (EEHRxF or X-Format), as put forward by the Regulation on the European Health Data Space sits at the core of primary use and is key to secondary use of health data. Without an EU-wide range of in-service demonstrators of X-Format use it does not “really exist”. This needs intelligent approaches to implementation through well-adapted IT-solutions and a multidisciplinary health professionals positive experience.

The i2X project has the overall goal of showcasing in-service, sustainable implementation of the X-Format, in all its data priority categories, in over 13 healthcare settings in 12 Member States involving more 40 partners. It aims to demonstrate that the EEHRxF improves individuals’ health and care journeys, without introducing undue burden on health professionals and systems. The main result will be 40 real-world in-service demonstrators that can inspire and guide all others to adopt the EEHRxF, improve adoption efficiency and produce lessons learned. Real world demonstrators that result from industry-driven and health and care organizations and professionals lead digital transformation that adds value.

This is an “implementation” project, where experimentation is encouraged in real life settings to demonstrate new/easier ways for data capturing and reprocessing aligned with format specifications and how patients are cared for. Making use of a set of advanced digital technologies that ease health professionals use of data i2X learn from its demonstrators and provide recommendations for pecifications and policy, while promoting standards alignment, wider awareness, and opportunities for non-partner organizations to also be early X-format adopters.

I2X relates to open innovation policy and implementation of MyHealth@EU services. It will build on previous projects and ongoing parallel EU initiatives such as Xt-EHR, XpanDH, xShare, while exploring synergies with other relevant merging consortia.

Gnomon Informatics SA is proud to announce another milestone! Gnomon leads a consortium that participates, as supplier, in the 3-phase iProcureSecurity PCP H2020 project which focuses on enhancing digital triage management systems using innovative technology packages. The affordable, personalized and based on well-established standards solution named iTRIER (standing for innovative Triage management system strengthening Resilience and Inter-operability of Emergency medical service), is proposed by the consortium to meet the project’s requirements. The consortium has successfully completed its first phase, where the focus was on solution design.

iTRIER: innovative Triage management system strengthening Resilience and Inter-operability of Emergency medical service

i-TRIER is the name both of the consortium led by Gnomon Informatics SA, and of the proposed innovative and affordable solution as part of the 3-phase iProcureSecurity PCP project; the latter aims to enhance digital triage management systems and has received funding from the European Union’s Horizon 2020 Research and Innovation Programme. The i-TRIER consortium includes market-leading industrial partners Agencies (CS GROUP-France, BETA80-Italy, ERC Grup-Turkey) with a strong record in integrating their solutions in the operational systems of Civil Protection, Ambulance Dispatch Centers and Hospitals, a well-established SME in the personal health data management domain (Gnomon-Greece), and two renowned research institutes (ICCS-Greece, and UPO-CRIMEDIM-Italy). i-TRIER aims to increase effectiveness and efficiency of emergency medical services (EMS) by developing a novel, modular, and highly flexible triage management system, which includes a fully digitized triage component, on-site management, control/decision support, and computer-aided dispatch systems with advanced capabilities for remote operations support. Additionally, it will provide strong interoperability for unifying emergency and victim tracking data with health data, and will support citizens by providing solutions for facilitating the access to their personal health records in case of emergency. The consortium has established an efficient work plan, and is continuously evolving its commercialization roadmap to ensure the solution finds its way into the operational systems of EMS and Civil Protection Agencies, ultimately contributing to their goal of saving lives.

UNICOM is about improved patient safety and better healthcare for all – it focuses on the implementation of the International Organization for Standardization (ISO) suite of IDMP (IDentification of Medicinal Products) standards.
Work will involve further development, testing, implementation and diffusion of these standards for:

regulatory purposes of national medicinal products authorities and the European Medicines Agency (EMA)

global pharmacovigilance

advancing cross-border digital health services, particularly ePrescription

better healthcare for all, public health services, clinical research, big data analytics, artificial intelligence applications.

UNICOM is dedicated to

improving patient safety globally

facilitating better healthcare for all

Patient safety issues, like medication errors, are due to numerous reasons, but many of them are related to a lack of readability of prescriptions, sometimes the partial absence of well detailed, structured and coded information on medicinal products – which also may somewhat differ depending on the national market, or missing information on other medicines taken by the patient.

The current digitalization of the healthcare sector may contribute substantially to reduce these threats, but without the adoption and concrete implementation of common conceptual and semantic standards to accurately identify and provide full, consistent and high quality data on all medicinal products available in the respective national or European market, the expected added value cannot be entirely delivered with data unable to travel between systems, regions, countries or use-cases.

All of this will be of great benefit to patients as well as healthcare professionals, and contribute to better healthcare for all across the Union.

The UNICOM Innovation Action has received funding from the European Union’s Horizon 2020 research and innovation programme under grant agreement No. 875299.
