Emergency Care or Care While Traveling

The Challenge:

People travel around the world but they can't use their health history in case of emergency. That's because firstly they don't have access, and secondly health information is scattered across different healthcare providers.

The solution:

eHealthPass gives to the patient all his health information gathered in a platform accessible from mobile or web. In case of an emergency, the patient can either show his personal health record (PHR) from his mobile phone or search affiliated specialists around him, get an appointment and share immediately his PHR.

The result:

1. Everyone can have his PHR wherever he goes,

2. Better insight of patient's health condition in no time, after patient has given consent for health care providers across countries.

3. The patient decides who and when can view his health information.

4. Improve patient-specialist relationship even if they don't know each other.

Family Health Management

The Challenge:

Even if everyone has the possibility to manage his own health data, some people can't. Elders are unfamiliar with technology, children under 18 or people mentally ill can't make legal decisions about themselves. Still they have the same rights in healthcare services.

The solution:

Close relatives will manage patient's health record such as children for the elders or parents for the children. Caregivers will have access to protected members from their own account.

The result:

1. The caregiver can track and update information in order to coordinate screenings and vaccinations that will prevent illness or lead to earlier diagnosis and better outcomes

2. The patient will have an up to date PHR after the caregiver adds information about him.

Chronic Disease Management

The Challenge:

Patients with multiple chronic conditions need to visit many different specialists. Each of those specialists will add different insights, treatments and recommendations to the patient's medical record. As more people are treated by multiple care teams, the need for greater access to these records increases.

The solution:

The patient is the only one to have attended all medical appointments, therefore he must get immediate access to an updated PHR. Specialists can add encounter reports or create treatment plans, patients record symptoms and vital signs such as blood pressure, weight measurement by using their compatible devices. eHealthPass gathers all this information and specialists will view them after a given consent.

The result:

1. Updated PHR for both patient and specialist.

2. Improved care coordination, by allowing patients to monitor various processes and view their health and treatment progress.

3. Improved observation of treatment plan's processes for specialists.
